Your question is important, I will precede that the definition according to Halacha of the iniquity of extracting sperm is indeed a very serious issue, but is not in any personal relationship that is not worthy of birth, but only when a physical means that prevents the penetration of sperm, such as a condom or diaphragm, and so on’.
Therefore, it is possible to prevent pregnancy without fear of sperm out, in several ways, as follows:

Using birth control pill.

Using an intrauterine contraceptive device.

Using a vaginal device – Novaring .

Using a weekly skincare – the Aura Sticker.

Using a three-month injection – Defoe-Probra.
I will note that in all of the above ways, the prevention rate is high at 98%, according to the doctors.
To the extent that the couple wishes to refrain from using any preparation, the only option in terms of Halacha is – the method of safe days. That is, calculation and planning is responsible for having a personal relationship only in the period of 13 days before receiving the menstrual cycle, which is a time that is not suitable for the absorption of pregnancy in most women.
The aforesaid is halachic, but medically speaking, before using any preparation, the recommendation of the obstetrician must be accepted.
